Outreach Health is a clinician-led adult healthcare practice being developed to improve access to care across North Carolina communities, with a focus on Mecklenburg County, Union County, Anson County, and surrounding areas.
Adult Primary Care
Preventive care, wellness visits, chronic disease management, medication review, lab review, men’s health, and specialist coordination.
Outreach Urology
Evaluation of common adult urology concerns including elevated PSA, urinary frequency and urgency, OAB, incontinence, recurrent UTI, stones, hematuria, catheter concerns, and men’s health.
Reach Aesthetics
Aesthetic consultations focused on natural-looking results, patient safety, and individualized treatment planning.
Payments
Outreach Health may use online payment processing for eligible self-pay services, aesthetic medicine services, consultation fees, deposits, administrative fees, or other approved patient payments.
Online payment does not establish a provider-patient relationship unless and until an appointment is confirmed and the patient is clinically accepted for care.
Refund and Cancellation Policy
Refunds and cancellations depend on the type of service purchased. For scheduled clinical or aesthetic appointments, cancellation and refund eligibility may depend on appointment timing, supplies, product preparation, and services already rendered.
If a payment was made in error or a service cannot be provided, Outreach Health will review the payment and issue a refund when appropriate.
Privacy
Please do not submit urgent medical concerns or detailed personal health information through general website contact methods. Secure medical intake options will be provided when online scheduling and patient intake are activated.
Emergency Notice
Outreach Health is not an emergency medical service. Online requests are reviewed during business hours and are not for emergencies. If you are experiencing chest pain, difficulty breathing, severe pain, stroke symptoms, suicidal thoughts, or another medical emergency, call 911 or go to the nearest emergency department.
